Wouldn't putting your name on the high score board in the arcade be considered the first? Well no I don't think so, because getting the high-score is the main goal in any arcade game. As I understand it, achievements are bonusses for deliberately doing something that is outside, or even opposite, of the actual goals of the game. For example in Pac-Man the goal is to eat all pellets, avoid or eat the ghosts and get the highest score. So this Pac-Man achievement on retroachievements.org is a good example I think, where the player has to deliberately leave the power pellets for last. Or the Poor Plumber achievement for Super Mario Bros, where you complete world 1 without collecting any coins, is also a good one (for the record; these achievements were not in the original games, they are added by using special emulators).

Achievements CAN be for doing unusual things, but that's not strictly a requirement. Many games have standard achievements for completing specific points in the storyline or hitting a certain character level. In all honesty, I kind of hate the ones that require you to do weird shit, because that's not the way the game was meant to be played.

Super Smash Bros Melee (or possibly the N64 game, I forget) would keep track of every bonus you've earned from completing a stage, and would only describe the ones you've earned in the past. SSB64 and Melee have bonuses. Brawl and SSB4 do not. To be precise, Smash Bros. 64 had end stage bonuses for fulfilling certain requirements in 1 player mode; while in Melee they incorporated these into multiplayer as well and let you keep track of which ones you've discovered. In both cases they gave (or took away) extra points while score in general was just for bragging rights, except in the one mode in Melee where score determined the winner.

Melee also had the trophies that you could get. Oh yeah! Brawl tied some of those to achievements. You could get hammers that would give you the trophy without performing the act. But IIRC the trohpies were unique, as in you could only get them there. No finding them in Subspace or in that shooting thing. I never thought of those as achievements until now. Of course the Xbox 360 was out well before Brawl came out, so it's a moot point.

The PS2 Ratchet and Clank games had "Skill Points" that could be collected by doing specific tasks, or doing things in a certain way
