It's less important to me now than it used to be, but back when I was a kid, the theme of a game was just as important as the actual game was. Many times, I skipped over these awesome games simply because I didn't like the hero I was supposed to be in control of.
A good example would be Sunsoft's Batman for the NES. Today, I regard it as one of the very best games on that system, but I declined buying it in favor of a game I consider not nearly as good now simply because I don't like Batman.
Another offender would be Blaster Master. If they would've stuck to the original theme (MetaFight), I would've bought it in a heartbeat. Instead some marketing dumbass thought it would be a good idea to insert this insulting and patronizing storyline about some kid chasing his pet frog.
Do you have any similar examples?

If we're discounting licensed properties, then not so much.
The weird thing about Blaster Master, though, is that the PS1 game later in the series took more after the English-language plot than it did the Japanese one...

Art style is big for me. I tend to stay away from titles that have a cartoony style or overly anime type art. I'm sure I missed out on some great games over the years, but those styles just don't appeal to me at all.

I'm sure it's a reasonably good game, but I can't stand the 90's Todd MacFarlane/World of Warcraft giant shoulder pads style.

I will say, though, one thing that can put me off is when a game gets a little too pervy, and gets blatant about it, whether it's in the game itself, or primarily in the advertisng... *cough* Ar Tornelico *cough*.

Yes, me too. That stuff is just too much for me. It also stems to games like Lollipop Chainsaw and Onechanbara. Just not for me.

I for a very long time had problems with 16 bit JRPGs.......because of the cutesy cartoony look i couldnt stomach the likes of chrono trigger, the 16 bit final fantasy titles or esp earthbound. im so glad i got around it finally.

I quit playing the God of War 1 demo as soon as you were forced to push a prisoner in a cage begging for his life onto a bonfire to proceed.

Mao, Sunsoft's Batman is barely EVEN a Batman game. I don't think a single person at Sunsoft saw the movie, and it shows. Like, why is it in the future? Why are there robots everywhere? Why does Batman shoot rockets? Why does the Joker shoot lightning? Seriously, it's pretty much the perfect Batman game for people who don't like Batman.
On topic:
- Moe characters. No matter the genre, a single one usually kills my interest in a game dead.
- I'm also really not fond of any sort of game that tries to shame you, the player. This has been a much more recent thing, but I get really annoyed by any game where you have to do bad things, and the game basically stops you to say "This is bad. The thing you are doing is bad. Stop doing the bad thing. Continue doing the bad thing to win the game." Spec Ops would be the most notable example, although I'm sure there are others, like MGS2.
- Bad pixel art. Now, there are plenty of games with good pixel art out there, like Freedom Planet or Retro City Rampage. I just hate it when developers, usually indie, make cheap, blocky sprites and make an art style off of that. Especially if they're trying to be "retro", where it comes off as more insulting, than anything.
- All right, this one is complicated: Something about all these games coming out lately that appeal very obviously to the Japanese market... something about them puts me off. These have been coming out for a while, now, usually RPGs, but lately, it seems like there's more and more. I can't give any specific names right now, but a lot of these games are usually very niche, at best, or they feel like they're appealing to somebody's specific fetish. And you know, if that's the kind of games people like, sure, whatever. I just feel like there's so many of them that they're starting to overpower the stuff from Japan that really interests me. Also, these usually tie in with my first point.
- Zombie games. All of them, forever. Stop. Stop it.

Sunset Overdrive is a game that, while I haven't played it, pretty much sums up what I don't like in games. I'm sure people will praise the hell out of it but the art direction, the "cool" attitude, the over-the-top DUDE gameplay.... It's a game by "cool dudes" for "cool dudes". I hate looking at it.

Ar Tonelico is particularly frustrating in this regard, there's a lot of cool concepts, but the devs seemed too busy writing bad innuendos rather than build on the game's genuinly creative ideas.

I think the company names you're looking for are Compile Heart and Idea Factory; and yes, it's disheartening that the Neptunia series in particular has been so successful despite its inherent nature as vapid, joyless bullshit for waifu-fawning neckbeards, and the insane amount of recycling and rehashing present across the trilogy which somehow goes excused or ignored.

The main character dressing in overly plain or practical clothes. If they look like something I'd wear on a day-to-day basis, I tend to lose interest. More annoyingly, this tends to be most common in third-person games, with most of the cool clothing and armor designs relegated to first-person titles. Also, shorts on main characters in Japanese games.
On that note, while not a complete turnoff, long coats that are stuck to the character's legs also annoy me. Devil May Cry had cloth physics and it was a first gen PS2 game, and even DmC's canned animations would be an improvement.
Also on that note, really short hair/shaved head/bald. 47 is an exception here, of course, but it just looks really lazy and boring how everyone seems too afraid to render hair.

There are few things more annoying than games that have some kind of character creation but only give you bald/extremely short hairstyles because their artists suck too much to make decent hair options.

I used to be very dismissive of games with giant robots, which for some reason I used to hate.
I am beginning to become more put off by games that rely a lot on cutscenes, especially when they're unskippable. I tend not to mind as much if they don't take themselves very seriously.
