Seeing this topic resurrected reminds me of a really stupid gaming mystery from my childhood that probably no one else cares about, as I'm sure very few other people have even noticed it, but which has fascinated me immensely for... some reason.
I speak of the soundtrack to Ironsword: Wizards & Warriors II.
I've explained this on multiple forums in the past (and I think I've explained it here in the Video Game Music section as well), so I'm just going to copy/paste from the comment I posted on this video:

I owned this game as a kid, and I'm 100% certain that in my copy, the title screen theme was Victory (9:34) and the first stage (Wind Mountain) theme was Store / Game Over (1:29). I no longer own that copy of the game, but I remember this with absolute clarity, and I've never been able to successfully confirm with anyone that there existed a variant of Ironsword featuring this particular music arrangement.
I did finally get confirmation of this from someone else who replied to that particular comment, but I've actually bought numerous $1 copies of Ironsword at conventions and such to see if I could find any with the swapped music, and to date every one of them has had the correct music usage (as labeled in the linked video).
...Which I say is correct, because even as a kid, I felt like the music choices in Ironsword were kind of bizarre and sounded very out-of-place. Later, when I learned what tracks were SUPPOSED to play on the title screen and in the Wind Mountain, it made a lot more sense, as those tracks are far better suited to those locations.
