Dragon Ball Z Budokai 1, 2, and 3 - When packaged together as Dragon Ball Z Budokai HD Collection (PS3/X360), the scores for all three games were replaced entirely, as composer Kenji Yamamoto had plagiarized most of the tracks from a multitude of other sources.

He scored Dragon Ball Kai as well...until his plagiarism got exposed and they had to rescore the show with the old Kikuchi tracks. I actually kinda liked the Yamamoto score for Kai, despite being plagiarized.

The Kikuchi tracks were horribly placed after Yamamoto got the boot, but fans in Japan didn't care; they welcomed back the Kikuchi score with wide open embrace.

A little surprised that this hasn't been mentioned, but the 1997 Sonic & Knuckles Collection (PC port of Sonic 3/S&K/S3&K) replaces the supposed Michael Jackson involved tracks with the original versions from the Genesis beta.

And no one knew that the "newer" tracks in the 1997 port were actually the original ones, until the Sonic 3 beta resurfaced.

Terry Big's Adventure - C64 clone of Great Giana Sisters (itself a clone of Super Mario Bros) that received a new soundtrack by Allister Brimble when ported to the Amiga.

Zoom - The US version of this Amiga game has completely different music from the PAL version, and the Genesis port features a third, still-different soundtrack.

Dune - The DOS and Sega CD version have a completely different soundtrack from the Amiga version, which received a completely new, Redbook score.

Wolverine: Adamantium Rage - The SNES version has a completely different soundtrack from the Genesis version. That's because they are two almost entirely different games who only happen to share the same title. Nothing unusual for licensed games.

Bishoujo Senshi Sailor Moon SNES & Megadrive are diffrent besides music from anime series.

I was looking at Jurassic Park for Amiga and PC (Adlib) (which are the same game, but different from versions on other platforms) and couldn't really tell if the music was the same or not. Amiga was more ambient, on PC I heard some melodies. I would tend towards them having different scores, i.e. not one being a rearrangement of the other. But again, I'm not sure. The MT-32 version appears to be the same as Adlib from the two tracks I found.
