Super Meat Boy - The 5th anniversary versions of the game (PS4, Vita, Wii U, and later Switch) have a completely new soundtrack, due to Danny B refusing to re-license his music to Team Meat. Also Binding of Isaac.

Teenage Mutant Ninja Turtles: Turtles in Time - As with TMNT Arcade in TMNT2: Battle Nexus, the unlockable Turtles in Time game in TMNT3: Mutant Nightmare replaces the music entirely with new tracks.

Looney Tunes: Space Race - For whatever reason, the Dreamcast and PS2 versions feature completely different soundtracks.

Crazy Taxi had a replacement soundtrack on the XBLA release since the license to the Offspring/Bad Religion had expired.

Superman (NES) - Kemco evidently had the rights to John Williams' Superman film score only in Japan, as the US replaces almost the entire soundtrack with tunes from Famicom RPG Indra no Hikari.

Rise of the Robots - An unusual case; Brian May was to compose the game's soundtrack, and did, but EMI decided to delay its release, and the game's devs were unwilling to delay the game to accommodate it. So, they had techno artists Fuzzy and Clownlogic make a new soundtrack. However, May's music eventually did appear in the 3DO version, which includes both soundtracks and allows the player to select which one they want to hear.

The Simpsons: Bart vs. the Space Mutants - The Genesis version replaces all of the music from the NES version. This includes removing the Simpsons theme tune, so licensing was probably the impetus here, though all the other music was thrown out too.

Epic Pinball - The first shareware version's soundtrack was replaced entirely for the later shareware version and full version.

Did you watch this week's Game Sack too? lol Speaking of Game Sack, I was watching an old episode (it's good background noise when I'm working) and the SNES and Genesis Star Trek: The Next Generation games seem to be very similar but they appear to have different soundtracks.

Spider-Man vs the Kingpin - The Sega CD update / remake/ remaster / whatever you want to call it throws out all of the Genesis version's music in favor of a new score by Spencer Nilsen; similar case to Sonic CD and Ecco.

Last Ninja 2 - The NES version, titled simply "The Last Ninja", doesn't use any of Matt Gray's C64 score.

Pengo - Surprisingly, many of the ports keep "Popcorn" as the in-game music, though the Atari 400/800 version replaces it with the Alfred J. Kwak theme, the classic version included in Pepenga Pengo for the Mega Drive changes it to an original track, and the European Game Gear version changes it to another original track.
