Ice Hockey (FDS/NES) - For whatever reason, the main gameplay theme from the FDS version (i.e., the track you hear for about 95% of the game) was replaced entirely for the NES version.

Someone said Nintendo supposedly treated the Japanese market as the beta grounds, and said that it was a smart move when you think about it.

NES Open Tournament Golf - For whatever reason, most of the music from the Japanese version, Mario Open Golf, was replaced for the US release. Furthermore, this game is something of a remake of two FDS golf games from 1987 (Family Computer Golf: Japan Course and U.S. Course), which have different music from either version of NOTG.

Might and Magic 3 had a few different OSTs over its many versions.
First is obviously the one used for the DOS, PC-98, Sega CD, Macintosh and probably a few other platforms i don't recall currently.
The SNES version, while different in terms of composition, still maintains a vaguely medieval style similar to the original. And then there's the PC Engine version which is, shall we say, much more modern sounding compared to the others. It's not completely out of place, considering the series' premise.

Dune II - The Genesis version, Dune: The Battle for Arrakis, has completely different music from the DOS version. This is somewhat of an edge case, since the game in general was overhauled considerably.

Speaking of Dune II, I just had a listen to the Amiga version, and while the intro music is the same as DOS, in-game there only seems to be one ambient track which is completely different from any of the PC tracks.

I just remembered that Blackthorne PC version soundtrack is completely different from the console versions (SNES / Sega 32x).
When it comes to the two console versions, they use the same songs, but a couple are switched in terms of the levels they play in, and some sound quite a bit different due to different instruments / arrangement. The 32x version lacks the final boss song - instead using the one from the previous set of levels.

Slap Fight MD -- An unusual same-version example, in that it contains both the original arcade soundtrack as well as a brand-new soundtrack composed by Yuzo Koshiro.
edit: nvm, the new soundtrack plays only during the "Special" mode, which is pretty much a completely new game.

Fantavision -- Unusual in that it has three completely different soundtracks, one each for Japan, North America, and Europe.
