I can only think of the one in Guilty Gear XX, I really liked that red monster thing popping up from the dark holding the words GAME OVER in its teeth.

Props to Weasel for mentioning I.Q. and I.Q. Final. No YouTube video that I can find, but trust me, it's got one hell of an awesome game over screen, with a big booming voice that yells "FAILURE!!". (:
Another vote goes to "Fatal Run" on the Atari 2600. One of my favorite game over screens of all time.

Persona 3's is probably the most haunting I can think of in recent. It's bad enough you more than likely just got a game over due to an extreme fluke(I.E. one lucky instant death spell), and then you have to deal with Igor lamenting your death with sad piano music playing in the background.
On a similar note, Wizardry: Tale of the Forsaken Land's is creepy. It shows about how you enter the underworld, leaving only the eternal dark in the town of Duhan.
Of course, even the game's actual ending is thoroughly bleak.

Another good one I just thought of: Shadowgate on the NES. Not just a creepy portrait of Death with a unique description for each individual way you could die, but also a really creepy dirge playing in the background. That was one hella atmospheric game. (:
