Grandia 2, if I remember correctly. Awesome, awesome battle themes in that game It does, indeed. The standard battle theme, and the "You were Surprised!" Battle theme... plus 2 boss themes counting the final boss. Further, and I hate to even make this claim, because everyone has their biases, but I'd say that soundtrack overall is probably one of the best I've ever heard. Everything just... fits with where it's used in the game.

Grandia 1 also changed battle themes halfway through the game and even changed it's battle and spell sound effects.
I think the best game for this however is the fantastic Panzer Dragoon Saga. Not only does that game have quite possibly the greatest collection of music ever for a videogame but each area usually had two different battle themes and there were around 15+ boss battle themes.

Well there is always FF8- Laguna and the main party had seperate battle themes. Same goes for Breath of Fire 4- different theme for the playable antagonist and the main bunch of chumps.

Skies of Arcadia had different battle music depending on if you were in a dungeon or on the world map. Also worth noting is that the boss battle music changed (in real time) depending on the situation, becoming more suspenseful if you're losing and more triumphant if you're winning.
7th Saga also had different battle themes for dungeon and map exploration.
Chrono Trigger was supposed to have a new standard battle theme after you reached the End of Time for the first time, but it was never used for some reason. It's still in the soundtrack.
Interesting note about Breath of Fire 1 and 2; the battle themes change about halfway through each game, but if you listen to the second battle themes from Breath of Fire 1 and 2, you'll notice that they're the same song. The version in BoF2 is a rock remix.
Also, Breath of Fire 4 has a different set of battle themes for Ryu and Fou Lu.
Baten Kaitos only has one normal battle theme, but it has a fuckton of boss battle themes that are all awesome.
The Legend of Dragoon had three different normal battle themes it alternated through at different points in the game.
Magna Carta has lots of normal battle themes and boss battle themes.
Radiata Stories has different battle themes depending on whether you're fighting monsters or other characters.
All of the more recent SaGa games have a shitload of battle themes. Unlimited has 15 different normal and boss battle themes.
The battle theme for Wild Arms V goes through some evolution, adding instruments as you progress through the game before it becomes complete.
That's about all I can think of right now.

Also, Breath of Fire 4 has a different set of battle themes for Ryu and Fou Lu. actually it just depends on which side of the map you are on. if you are on the West, where Fou Lu starts and where you play as him, it has different music than where Ryu starts. but once you cross over to western side of the map with Ryu, it starts playing the other battle music. though it could be i'm wrong, i dont think you can have Fou Lu cross over to the Eastern parts of the world to find out if this is how it works. it's been a while since i've played it.
