There's Dark Messiah (JP title) or as known in PAL regions, Hellnight. The setting is an underground system of tunnels and sewers which the main character finds himself in after an accident.
Have you heard of Slender? Do you know all these indie horror games lately which are heavily inspired by Slender? Well, Dark Messiah did the whole first-person RUN FOR GOD'S SAKE RUN horror already back in 1998 as you explore the underground while being chased by one hell of a scary monster. The sound design is absolutely fantastic, and it is extremely intense. While you have a companion (not visible, in-game he exists inside you, which is actually great since there's no stupid AI pathfinding bullshit) at the beginning who grants you the ability to identify the monster on an automap if its nearby, I DARE you to play the game without her. Maybe it becomes a bit unfair without her aid, but dear lord does it get nightmarishly tense. Or well, it is scary regardless. PLAY. IT.

I'm still in the midst of playing the game though, so I'll avoid it until I'm done, since HG101 articles are generally infested with spoilers.

Hope these gonna have any relevance,since the thread is not exactly new: Covert Ops: Nuclear Dawn (PSX)- Resident Evil on the train.You get to fight 'shooty' terrorists instead of zombies tho; Cold Fear (Multi)-the overall ambience of the game reminds me of the 1st RE Revelations a lot; Mystic Nights (PS2)-apparently released only in South Korea.I barely touched it,but the first impression i've got: it's entirely 3rd person Resident Evil:Dead Aim lookalike without first person shooting bits; Banned Memories: Yamanashi-unfortunately i can't say much about it at all,other than it's still in 'Early Access' and aims for original Silent Hill look & feel; Daymare: 1998--this one's from folks who didn't have much luck with Resident Evil 2 Fan Remake.The Kickstarter campaign has been canceled literally hours ago ,but they said,they won't be givin' up on it just yet !

Adding a few that I don't think have been mentioned:
Escape from Bug Island (Wii) - released in 2007, this is maybe the last survival horror game that feels as if it's made in the late 90s/early 2000s, it sorta has that ridiculous blue stinger/illbleed cheap look and feel.
Zaphie 2 (PC) - Korean Resident Evil/Silent Hill merger, has a great feel to it and just the right look, Zaphie 1 is Myst with a Resident Evil style, not as cool.
Nanatsu no Hikan: Senritsu no Bishou AKA Seven Mansions (DC) - Japan only game for the Dreamcast that usually doesn't get mentioned in survival horror lists.
Dinosaur Hunting (Xbox), this one is borderline cause it's not quite survival horror, only released in Japan on the original Xbox, US release seems to have been canned in the last second, maybe they saw the sales numbers of Dino Crisis 3 and said nope.

Lol, that's my Zaphie 2 longplay I gave up on. Also having played Escape from Bug Island I can attest to it feeling like a cheap PS2 game. The game feels like something outta the Simple series. I've heard of Seven Mansions, but this is the first time seeing gameplay of it. The music sounds so disturbing; really wish it had been brought to the US. Never heard of that Dinosaur hunting game.

Honestly, Bug Island didn't even register much as a Survival Horror game to me when I played it, despite its marketing. The game felt kind of Arcade-y and mostly tries to be a cheap 50s B monster movie. Like Simple 2000 puts it pretty accurately. Though it is a bit meatier and more fun thant those are - could use a nicer control scheme, tho.

That's the nightclub vampire game where you can choose to finish off your enemies permanently if you want to. It's an okay game, and has quite a bit if replay value for it's endings if I recall correctly. Aside from that I don't remember much from my playthrough.

I don't know how relevant is this Checklist thread-chances are, there's a new horror themed game coming out every day. Maid of Sker looks scary, YT asks you to sign in for age confirmation.

You also got Vampire Hunter D if you want something with vampires after you've played through Countdown Vampires. Also of interesting note is Soul of the Samurai AKA Ronin Blade, I wonder if the 3d engine konami used for this game is based on Silent Hill or MGS cause it feels like somebody reskinned Resident Evil with a Samurai theme, I like this game a lot. Also while googling I saw this, a Countdown Vampires comic book, great cover: Also there's one thing new survival horror games really miss and that's tribal tattoos!, I wonder if the Resident Evil Zero team is doing some nodding towards Countdown Vampires or if it's just a zeitgeist thing.

Maybe not obscure, as such, but has anyone mentioned Extermination on Playstation 2 yet?
It is effectively The Thing as a computer game... Before The Thing became a game of its own on Playstation 2 anyway.

Also there's one thing new survival horror games really miss and that's tribal tattoos!, I wonder if the Resident Evil Zero team is doing some nodding towards Countdown Vampires or if it's just a zeitgeist thing. I'm pretty sure it's just a late 90s/early 2000s thing. I've also always thought the RE0 dude's tattoo says "mother love"
