I'd like to help, but I don't know where to start. Which patch? What patching application? What emulator? etc...
Anyway, in general your rom needs to be the exact correct file size down to the byte in order for a patch to be applied to it. Usually, a readme comes with a patch telling you what size and region (US, JPN, EU) it needs to be. Sometimes emulators apply the patch automatically. Just in case name each file, the rom and the patch, the exact same (leaving the file extension as it is).

Alright, I'll try that stuff
The patch is for the PSII JP version, with Kega Fusion as the emulator and IPS as the patch.

What was the patch for? A retranslation? Just curious.

Yeah, because apparently the original translation sucks, so I decided I'd try this!

Get ready to turn the volume down; the music in the Japanese version of PS II is hilariously terrible. Think of all those catchy tunes you loved in the US version. Now, add an earth-shattering CRACKING POUNDING DRUM SLAM every other beat and that's about whatcha get. And it's in every song... and it doesn't stop. Ever. That drumbeat is musical cancer.

Just play the NA release man. The translation isn't that bad, and the music is better for sure. Have you played the first game at all?

Hunt down the Phantasy Star collection cart for GBA. I think you will enjoy it.

I thought it had, like, fatal errors?

Yeah - occasionally PS1 will crash when you save -_-;
I keep meaning to play through the Jp version of PS4 as that apparently has some significant changes to the script in some quite important places too.
