Elite Beat Agents brought this to my mind (the American version of Osu! Tatakae! Ouendan!), so I'm trying to make a list of games that went some drastic localizations during their trips overseas. Not small stuff, like removing blood or anything, but complete graphic overhauls, new characters, license tie-ins, etc. Here's what I've come up with so far:

Nekketsu Kouha Kunio-kun / Renegade (Kunio/Double Dragon 2 GBA)
Super Mario Bros. 2 / Doki Doki Panic

Black Belt / Hokuto no Ken - Last Battle / Hokuto no Ken 2
Blaster Master / Meta Fight (?? Only kinda sorta, the only thing that really changed was the premise/intro)

Tetris Attack / Panel de Pon
Wario Blast / Bomberman (Japanese GB)
if I remember correctly, all the Kemco made Bugs Bunny: Crazy Castle games originally Disney licensed games in Japan. Capcom of America hold an exclusive on Disney games, so they had to change all the sprites and characters into Looney Tunes.

Yeah. might as well add Pokemon Puzzle League in there too. Didn't know about the Wario game though.
Also, this is kind of a stretch, but the premise in the arcade version of Astyanax is a lot different than the home version (and mostly a different game altogether.) The Japanese version is hilariously titled "The Lord of King".
If we consider that, there's also the Wonder Boy/Adventure Island, plus Saiyuki World debacle. Those aren't really due to localization though.

Yeah. might as well add Pokemon Puzzle League in there too. Didn't know about the Wario game though. was there ever a Panel de Pon game for the N64 in Japan and was changed over here to Pokemon Puzzle League? I always assumed that America/Europe was the only place that got Pokemon Puzzle League. Wario Blast is kind of pathetic, as the only addition to the game is the inclusion of a Wario sprite. That's it.

And if you want to go that route, then Top Secret / Bionic Commando counts.

That would make an awesome article.
BTW, Elite Beat Agents is a whole new game and not necessarily a localized Ouendan. You could call it a sequel if you want.

I think La Pucelle is radically changed considering Discoalucard mentioned that all of the Christian imagery was left out in his article. It totally changes the mood of the game. Also, all of the early Dragon Warrior and Castlevania games had that same thing happen to them when they were localized. And while we're talking about Panel de Pon, you guys should import Nintendo Puzzle Collection if you have a Freeloader for the GC. It has one of the best versions of Panel de Pon on it with all of the extra characters, 3D puzzle mode, and, best of all, 4-player mode. Get it from Play-Asia if you can.

Maniac Mansion for NES/Famicom comes to mind - the American NES version was different from the PC version already, but the Famicom version was actually a complete overhaul of the game. Graphics are entirely different (making it almost seem like a whole different game), there is no scrolling...
Then there's Snatcher. Katrina's outfit changed, and all kinds of other stuff was modified, like the addition of the Konami bios on JORDAN (including Jeremy Blaustein, the translator, who you can call on the game's video phone a few times).

Capcom's Sengoku Basara / Devil Kings
Not only did they change everyone's names from historical figures to comic book sounding stuff (Masamune Date --> "Azure Dragon"), and change most of the graphics, they made 4 characters unplayable.
Hiroyuki Kobyashi butchered his own game for NA, and not CoA, like many people think. >_>

I don't think it's been mentioned: Ace Combat 3: Electrosphere. Upon english release, it had all of the characters, all of the cutscenes, all of the voice acting, all of the animated segments, the intro, the branching paths and about half of the missions cut. I think some of the planes were removed, as well. It was all replaced with pre-rendered videos of text fading into view on a static background with a voiceover.

A more obscure one is the official Korean releases of some Samurai Shodown games. They're called Fighter Swords, many of the names are changed to Korean names, and many of the voice overs are altered and/or removed entirely.

The entire Megaman series, most of the important characters' names were changed; Dr. Light got a number of changes to his name, the whole Rockman/Megaman thing, Blues got Break Man and Protoman, Vava vs. Vile, and I can go on and on. Also of note was that CoE are jerks when it comes to packaging Megaman games.
