Some older games, like Persona 2: Eternal Punishment on the PS1, did not allow the in-game clock to go past 99:59, so I put over 100 hours into the game, but don't know how many.
I out over 100 hours into Dissidia and Disgaea 2 on the PSP.

I'm a man who rarely breaks 40 hours on a single game. But I put just under 115 into Shin Megami Tensei: Nocturne. That is, without a doubt, the longest I've ever spent on one title.

As far as retro games go: Paladin's Quest (no surprise there, I'm sure), Final Fantasy VI, Chrono Trigger, Chrono Cross, Super Metroid, Mario Kart and Mario Kart 64, Final Fantasy Tactics Advance, Doom and Doom II, Star Wars : Knights of the Old Republic and Star Wars Knights of the Old Republic II: The Sith Lords, to name a few.
And as far as modern-ish games go: Minecraft, The Elder Scrolls V: Skyrim, X-Com: Enemy Unknown, and Mount & Blade: Warband.
Some of those games have had a lot of replay value for me due to various mods, fan-made expansions and total conversions though.

Diablo 2 is undisputed king for me. I've been playing it for years. Most recently - Gems of War. I've started it around August last year and still play it pretty much everyday, hard to say how many hours i accumulated already, must be a LOT! I also had been playing a few Pro Evolution Soccer titles extensively with a friend, but have since abandoned the idea. Always hated FIFA Played Resident Evil Revelations Raid Mode enough to burn out on it. Still haven't finished it completely.

Across various modes, I've somehow put almost 100 hours into Resident Evil 6. :S
I still don't know if I like it that much. It's such a weird game because the quality of the campaigns varies wildly.

My number one most played game ever kind of depends on how you count it. If you see the 3DS and Wii U versions of Smash 4 essentially as two versions of one game, that's probably the one. If you consider those seperately, Splatoon 2 is probably number one with 430 hours currently played. Splatoon 1 has at least over 300 hours as well. Melee and Brawl are in that range as well.
I've also played a lot of Spelunky and Minecraft on PC over the years. Those must be pretty high up there too.
Back in GCN days, there were a few multiplayer titles we would always whip out. So Mario Kart: Double Dash and Mario Party 5 must be among my most played ever as well.

Hard to tell with games I played more than once (which doesn't happen that often, tho), but despite usually being on the quick side and considering a game already really long when it hits the 60h, somehow I already put about 130 into Monster Hunter World and my 3DS tells me I put a combined 400+ into New Leaf. Sims 3 would probably have also added up to quite a bit, but I have no way to tell.

My guess is Might and Magic VI, but that doesn't have a timer. That was my main game for a very long time and I didn't even get to finish it! I got the strategy guide after a few months, and went through absolutely everything I could. I finished almost every quest, opened every single chest and cleared out each map of enemies. Enemies do respawn after a certain longish amount of game time, so I went back to some of the earlier maps and recleared them for good measure. My hard drive crashed before I got a chance to actually finish it though 😢
For single plays of games with a timer, Xenoblade 2 at ~105h. I think that's my only 100+. XBX was pretty close though at 95h. I still had plenty of stuff to do too. SMTIV is #3 at 90h.
For total time, I think it'd be the original Xenoblade. I put in at least 200 hours for my 3 longest playthroughs of it. My Wii was one of the earlier ones that ended up having trouble reading dual layer discs. There's about 10 dual layer Wii games, I have 5 of them. MP Trilogy and Metroid: OM never game me any trouble. SSB Brawl only worked if the system was level; some friends' old warped floors caused some trouble, but I never had issues at home. The Last Story was a little iffy once or twice, but had no problems otherwise.
Xenoblade though, barely even worked. I replaced the disc twice before the guy at GameStop said that the odds of three bad discs in one shipment was infinitesimal. I soon learned that Nintendo made a laser cleaner and that did help a little bit. I'd give the laser a good polish and then play it for about for five to ten hours. This is probably why I finished XCX and XB2 in such short time spans (3 weeks each). That only worked until the Sartorl Marsh when it just refused to work at all after that point 😕 I was afraid LS would be the same way after having a little trouble at first, but it ended up fine. I restarted when I got a Wii U and played it again when the 3DS version came out.
The other possibility is Twilight Princess because I'm finished it so many times (5 or 6).

If you wanted to combine the various Street Fighter 2 incarnations, then it's probably that.
Now if we are talking about a single game...that's hard for me to pin down. I've played short games, like Mega Man 2 dozens of times but long games like I dunno, Dragon Quest VIII only once.
Starcraft (consumed my life in high school), Third Strike (consumed my life in college), or Earthbound (play through it every other year or so) would probably be my most played.

As far as games I've replayed the most and not just hours put into that would have to be Silent Hill 3 and Metal Gear Solid 1 and 2.
As a teen I would play those games the way you'd watch a favorite movie, over and over again, I got to where I could beat them in one sitting (not all at the same time, just individually)
I've slowed way down on that though, I haven't played any of the Metal Gears pre-Phantom Pain since 2011 and since 2010 I've only given Silent Hill 3 one playthrough, I just don't have the patience to replay games very frequently anymore, plus back then I would replay those games when I didn't have anything else to play, since these days I have more games than I know what to do with frequent replays is not much needed.

Highest hours for a single game would consists of: Persona 5 - 80+ hours Xenogears - 54+ hours
Monster Hunter World seems like the kind of game where you need to spend over 200+ hours. My friend seems to have spent 400+ hours on it, alone.
As for games, that would be cumulative from replaying? Final Fantasy VI and Chrono Trigger would top them both. I've lost count how many times I've replayed them, but surely I've logged in over 100+ hours for each. I usually take about 16~20 hours for each game, depending on pace.
