Lords of Thunder, the first game that came to mind, probably doesn't count. Its alternative soundtrack keeps the same, basic themes but changes instrumentation. I always wondered why, because there was no technical reason for doing so (both soundtracks being blue-book audio), and the original already sounded great.

Sailor Moon (1993) - Super Famicom beat-em-up based on the anime's first season. For whatever reason, when the game was ported to the Mega Drive, all of the music was replaced by new tracks (as were all of the cutscenes.)
